Healthcare in Hungary 2026: NEAK, EHO & cover for expats
How Hungary's public health insurance works for residents, visitors and newcomers, and where it falls short.

Hungary's public health insurance (NEAK) covers about 96% of residents through payroll contributions, government-paid categories and a flat monthly contribution. In 2026 that contribution (EHO) is HUF 12,300 per month for eligible uninsured residents after a year of registered residence. EU visitors use the EHIC; non-EU permit applicants must show comprehensive cover or means. Elective waits and out-of-pocket costs are the main weaknesses.
How healthcare in Hungary works
Hungary runs a public, contribution-based national health insurance system administered through NEAK, the National Health Insurance Fund Manager. According to the OECD/EU Country Health Profile 2025 for Hungary (finalised in September 2025), the system provides coverage for about 96% of the population, leaving around 4% of people without clear insurance status, for example people working abroad or without a permanent address. Entitlement rests on residence, meaning people with a permanent registered residence for at least 12 months, plus mandatory contributions.
This guide covers how the public system works for someone who moves to Hungary, what it costs, how visitors and non-EU residents are treated, and where it falls short. As of September 2026: the figures below were checked against NEAK, NAV, the immigration authority (OIF), the European Commission and the Country Health Profile in late September 2026. Health rules and amounts are updated regularly, so confirm anything you rely on with the authority directly.
Who is insured
The Country Health Profile describes four routes into the public system:
- Economically active people, who are covered through their contributions.
- People whose contributions are paid by the government, for example children under 18.
- People covered through international agreements, such as those with cover from another country's system.
- People covered through a monthly flat-rate contribution to the health insurance fund, which is the route the EHO section below describes.
For social security purposes, NEAK defines "domestic" persons as Hungarian citizens with a registered residence in Hungary, people with permanent-residence entitlement, recognised refugees and protected persons, stateless persons, and people with free-movement and residence rights who hold a registered residence address. This definition matters because the flat-rate route is tied to it.
Employee contributions
If you work for a Hungarian employer, health insurance comes through payroll. NAV lists the employee's social security contribution at 18.5% in 2026, split as follows. PwC's tax summary also gives 18.5% for employees and an employer social contribution tax of 13% in 2026.

The EHO: the monthly contribution for uninsured residents
The health service contribution (egészségügyi szolgáltatási járulék, EHO) is for people who are not otherwise insured. NEAK says it must be paid by a domestic-status person who is not insured and has no entitlement to health services on any other basis. Paying it gives entitlement to the full range of the health insurance system's health services. NEAK's wording concerns health services, so treat it as access to care rather than assuming it carries other benefits.

A few practical details from NEAK's page are worth knowing. After insurance ends, a person whose earlier insurance lasted at least 45 days keeps a "passive right" to care, and the EHO obligation starts from day 46. People whose entitlement rests on the EHO itself, or on an agreement, do not get a further 45-day passive right when that ends. NAV sends a notice within 8 days of an insurance ending and posts the liability automatically. If that does not happen, the person must register with NAV on form 08E and declare that they have no insurance in another EEA or social-security-agreement state.
NEAK also counts Hungarian nationals who have lived in Hungary for at least a year and did not notify foreign insurance under Act LXXXIII of 1997 as domestic for EHO purposes. The main laws are Act CXXII of 2019 for entitlement and Act LXXXIII of 1997 for health insurance benefits. NAV and PwC both confirm the HUF 12,300 monthly amount for 2026.
EU and EEA visitors: the EHIC
Visitors from EU and EEA countries use the European Health Insurance Card (EHIC), which is issued free by your home insurer. The European Commission's Hungary page sets out the practical rules:
- You must consult healthcare professionals under contract to NEAK.
- Consultations are free of charge, and specialists require a referral.
- A fee is charged for prescribed medicines, either a reduced fee or the full price depending on the medication.
- Certain dental treatments carry charges outside protected groups.
The EHIC does not cover everything. Your Europe, the EU citizens' portal, notes that if you cannot use your card, for instance for private healthcare, you cannot be refused treatment but you might have to pay the full amount. The card is meant for temporary stays, not for settling in Hungary. If you move permanently, you would come under Hungarian rules or those of the state that continues to insure you.
Non-EU residents and permit insurance
For third-country nationals, health cover is part of the residence permit process. The immigration authority's factsheet for the National Residence Card lists housing, subsistence and health insurance among the requirements. It says the applicant must be considered to have access to comprehensive health insurance services covering all healthcare services, or be able to cover the costs of such services. That can be satisfied through social security cover, international agreements, or documented financial means.
We did not find a primary source that names a particular type of insurer or policy, and the factsheet was reviewed through a summarising tool, so the exact wording for your permit type should be checked with OIF. How the system performs
The figures in this table come from the OECD/EU Country Health Profile 2025 for Hungary. EU averages are the profile's own comparators.

The profile presents a mixed picture. Reported unmet need is lower than the EU average, yet spending is around half the EU average, staffing is below it, and life expectancy is 4.7 years below it. Public financing accounts for 74% of current health spending, and voluntary health insurance plays a minor role at 3% of health spending. Private providers play a minor role in secondary and tertiary care, and GP services are mainly delivered by individual private practices under local government oversight.
Everyone in Hungary, whatever their status or contributions, is entitled to a limited set of publicly financed services: preventive care for communicable diseases, emergency transport and emergency care for 31 diseases and conditions. For emergencies, the EU-wide number 112 works in Hungary.
Challenges and downsides
- Long waits for elective surgery. The Country Health Profile reports that in 2024, 29% of patients waited more than three months for cataract surgery (7% in 2019), 74% for a hip replacement (58% in 2019) and 78% for a knee replacement (65% in 2019).
- Out-of-pocket costs. Outpatient and pharmaceutical costs are only partly covered. The profile says the majority of pharmaceutical spending comes from out-of-pocket payments (51% in 2023), and that out-of-pocket payments are well above the EU average of 16%. Dental care is not fully covered either.
- Fewer doctors and nurses. At 3.6 doctors and 5.5 nurses per 1,000 population, Hungary is below the EU averages of 4.3 and 8.5.
- Informal payments. The Country Health Profile says informal payments have historically been a concern. It adds that the government more than doubled physicians' salaries over a three-year period starting in 2021 and criminalised giving and accepting informal payments, except for small-value gifts.
- Losing access through arrears. If EHO arrears exceed HUF 73,800, NEAK says the person's TAJ number is invalidated for health services, meaning they cannot use them free of charge until the debt is settled or a payment arrangement is made.
- Falling between the cracks. Newly arrived foreigners may fall into the roughly 4% without clear insurance status. Without contributions, the free entitlement is limited to emergency transport and emergency care for 31 conditions.
- EHIC limits. The card works only with NEAK-contracted providers, not at private ones.
- Health outcomes. Life expectancy of 77.0 years in 2024 is 4.7 years below the EU average.
Official steps and where to verify
- Employees: your employer registers you and payroll deducts the contributions; ask HR to confirm your insured status.
- Uninsured residents: read NEAK's EHO page and, if NAV has not notified you, register on form 08E with NAV.
- Non-EU applicants: confirm the health insurance requirement for your permit type with OIF before applying.
- EU/EEA visitors: carry your EHIC and use NEAK-contracted providers.
- Everyone: check current EHO amounts with NEAK or NAV, since the 2026 figure is dated January 2026.
